Zgłaszający powinni przedstawić:
- akt zgonu
- informację o przyjęciu sakramentu (zwykle jest to obrazek jaki zmarły otrzymał od kapłana)
- miejsce i czas pogrzebu
Jeśli zmarły jest z innej parafii rodzina powinna przynieść od księdza proboszcza pisemną zgodę i informację o zmarłym, który ma prawo do pogrzebu chrześcijańskiego.

All funeral arrangements are handled by the family.
A person reporting the death should present:
- the death certificate
- information regarding the acceptance of the sacrament (usually a picture handed over by a priest prior to the person’s death)
- the place and time of the funeral
If the late member of the family is from another parish, the family should bring written permission from the parish priest and information about the person who has the right to a Christian burial.
